No more images/media?

For the past week or so my options to add images, media, etc have all been greyed out.

Can't figure out how to turn it back on.

Any help?

It happens to me occasionally. I press the 3 vertical dots on the upper right of the text box next to the preview icon.

Then press [ ] and it opens up the options.
