I agree Ghana were really really unlucky on that incident but I don't think there's that much wrong with the rules. In many cases if given choice people would take a penalty and an opponent sent off instead of a goal and a yellow for example. The situations where this becomes an issue are so rare it makes it a very marginal problem but perhaps there could be a rule of awarding a goal if it happens during the last 10 mins? At least in major tournaments where that one goal can be so much more valuable.
